PGY1 & 2-Health System Administration & Leadership

Aimed at training the future leaders of integrated health systems, this 24-month program combines the PGY1 Pharmacy Columbia St. Mary’s residency with the Ascension Wisconsin PGY2 Health System Administration and Leadership residency.

PGY1 Program Purpose: PGY1 pharmacy residency programs build on pharmacy education and outcomes to contribute to the development of clinical pharmacists responsible for medication- related care of patients with a wide range of conditions, eligible for board certification, and eligible for postgraduate year two (PGY2) pharmacy residency training. During the PGY1 year, the resident will spend the majority of their time at Ascension Columbia St. Mary’s Milwaukee and Ascension St. Joseph hospitals, with the opportunity to spend time at other Ascension Wisconsin hospitals in the Milwaukee area.

PGY2 Program Purpose: PGY2 pharmacy residency programs build on Doctor of Pharmacy (Pharm.D.) education and PGY1 pharmacy residency programs to contribute to the development of clinical pharmacists in specialized areas of practice. PGY2 residencies provide residents with opportunities to function independently as practitioners by conceptualizing and integrating accumulated experience and knowledge and incorporating both into the provision of patient care or other advanced practice settings. Residents who successfully complete an accredited PGY2 pharmacy residency are prepared for advanced patient care, academic, or other specialized positions, along with board certification, if available. The PGY2 year builds on the clinical foundation developed during the PGY1 year to develop pharmacists capable of being future leaders of integrated health systems.

OVERVIEW
During the PGY1 year, residents complete core learning experiences in administration (medication safety, informatics), direct patient care (medical/surgical, emergency medicine, critical care, women’s health/NICU), academia, research and staffing. During the PGY2 year, the format of the residency is tailored to the interest, experience, aptitude and career goals of the resident. This residency takes a unique approach where the resident participates in rotations, projects and committees that are based at the state and national level. The rotations are uniquely customized to meet the specific career development needs of the resident. Moreover, during the PGY2 year the resident participates in the Pharmacy Society of Wisconsin’s Pharmacy Advancement Leadership team. After the completion of the program, the resident is well rounded and eligible for leadership positions of supervisor, manager, director or equivalent.

Dr. Mazen Samadi is a pharmacist from the Milwaukee area. He received a B.S. in Pharmacy with a Minor in General Business from the Lebanese American University in 2016. He subsequently received his Doctor of Pharmacy from the Lebanese American University in 2017. He has since traveled and moved to the United States in pursuit of an in-depth pharmacy experience. He has thus far completed a PGY-1 Pharmacy Practice Residency at Ascension St Francis Hospital in 2018. His multi-variate interests include: internal medicine, medication safety and clinical management. Currently, he is pursuing the PGY-2 Health System Pharmacy residency for Ascension Wisconsin to help widen his experiences and leadership. Ultimately, he wishes to integrate into a healthcare system or hospital as a clinical coordinator or in a position related to clinical management. Mazen is also seeking to bolster his credentialing and networking by seeking board certification and involving himself more in the International Pharmaceutical Federation (FIP) during this upcoming year.

  • Brook DesRivieres, PharmD, MS
    Dr. DesRivieres is a Regional Director of Pharmacy for Ascension Wisconsin in the Milwaukee area. She serves Columbia St. Mary’s – Milwaukee, Columbia St. Mary’s – Ozaukee, Wheaton Franciscan – St. Joseph’s Hospital and Wheaton Franciscan – St. Joseph’s Outpatient Center.

    She obtained her Doctor of Pharmacy from the University of Michigan, master's degree in Health-System Pharmacy from The Ohio State University and ASHP-accredited PGY1 residency training from Grant Medical Center (OhioHealth).

    She serves on the American Society of Health-System Pharmacists (ASHP) Section of Practice Managers Education Steering Committee and is the past chair of the Section Advisory Group on Patient Care Quality. As the Residency Program Director for Ascension Wisconsin’s PGY2 in Health-System Pharmacy Administration, she precepts the longitudinal Leadership rotation.

  • Christopher Dykstra, PharmD, MBA
    Christopher Dykstra, PharmD, MBA received his PharmD from Drake University in Des Moines, Iowa. He went on later to receive his MBA from Lakeland University.

    Chris is the Regional Director of Pharmacy for the Milwaukee South Region of Ascension Wisconsin. Prior to this role, Chris was the Pharmacy Operations Manager and supervisor at St. Joseph hospital. Chris was the preceptor for the required PGY1 pharmacy administration rotation as well as the longitudinal Anemia Management Clinic rotation.

    Chris is a member of the Pharmacy Society of Wisconsin and American Society of Health-System Pharmacists. His areas of interest include advancement of hospital pharmacy practice, through technician development and expansion of responsibilities as well as pharmacy education. He currently serves as preceptor for the informatics rotation within the PGY2 Health System Pharmacy Administrative Residency.

    Chris is married to his wife, Sarah, and has three children, Allison, Nathan and Eleanor.
  • Diane Erdman, PHARMD, BCPS, CDE, CPPS, BCACP
    Diane M. Erdman, PharmD, BCPS, CDE, CPPS, BCACP received her Doctor of Pharmacy degree from the University of Wisconsin - Madison School of Pharmacy and completed a specialty residency in primary care at the University of Oklahoma College Of Pharmacy in Oklahoma City. She is board certified in pharmacotherapy and ambulatory care, is a certified diabetes educator and a certified professional in patient safety.
    Diane started her clinical pharmacy career after residency at Grady Health System in Atlanta and then took a position at the Lawrence Joel Army Health Clinic at Fort McPherson as the only clinical pharmacy specialist. There her practice included disease statement management, initiating diabetes classes and anticoagulation clinic, medication safety, and formulary management. In 2005, Diane joined Kaiser Permanente-Georgia in Atlanta as a clinical pharmacy specialist providing diabetes, hypertension and lipid management and became a clinical pharmacy manager with oversight of the CKD, ID, Hepatitis C, oncology and acute care pharmacy services in 2008. She also served as the residency program director for the PGY1 Managed Care and PGY2 Ambulatory Care programs from 2007-2015.

    In February 2015, Diane joined St. Joseph Campus in Milwaukee as the pharmacy clinical manager and residency program director for the PGY1 residency program. Her practice interests include diabetes management, medication safety and quality improvement. She is a member of a number of professional organizations and serves as a clinician surveyor for ASHP PGY1 Managed Care and PGY2 Ambulatory Care residency accreditation surveys.

  • Mike Gillard, PHARMD, BCPS
    Mike Gillard, PharmD, BCPS received his Bachelor of Science in pharmacy from the University of Wisconsin - Madison in 1993, completed a PGY-1 Pharmacy Practice Residency at the University of Wisconsin Hospital and Clinics in 1994, and earned his PharmD from the University of Wisconsin - Madison in 1996. He became board certified as a Pharmacotherapy Specialist in 1997, and he currently holds that distinction through 2018.

    Mike has worked in both clinical and leadership pharmacy positions, most recently as the clinical pharmacy supervisor for Wheaton Franciscan Healthcare - Franklin since 2008. From 1999 to 2002, Mike served as a preceptor, primary preceptor and Residency Program Coordinator for the PGY-1 Pharmacy Residency Program at Aurora Health Care. Mike also served as acting Pharmacy Residency Program Director for the PGY-1 Pharmacy Residency Program at Wheaton Franciscan - St. Joseph Campus from 2011 to 2013. He was a United States lead in the Medication Reconciliation project, a component of the High 5s initiative; a project sponsored by the World Health Organization, Agency for Healthcare Research and Quality, The Joint Commission, and Joint Commission International which seeks to improve patient safety around the world.

    Mike is a member of the American College of Clinical Pharmacy, the Pharmacy Society of Wisconsin, serving on the board as Region F Director from September 2014-present, and the American Society of Health-System Pharmacists. His areas of interest include medication policy, medication safety, pharmacy informatics and advancement of hospital pharmacy practice through leadership, staff development, and pharmacy resident and student education.

  • Justin Guthman, PharmD
    Justin Guthman, PharmD, received his PharmD from the University of Minnesota (Duluth) College of Pharmacy. Following pharmacy school, he completed an ASHP-accredited PGY1 Community Pharmacy Residency and a PGY2 Health System Pharmacy Administration Residency from Froedtert Hospital (Milwaukee, WI).

    Justin has worked in a variety of leadership pharmacy positions, most recently as the Regional Director of Pharmacy for the North Region of Ascension Wisconsin. Prior to this role, Justin was Director of Pharmacy at two Ascension hospitals and a retail pharmacy. In 2017, Justin worked with the Ministry Saint Clare's team to establish a PGY1 pharmacy residency program.

    Justin is a member of the American Society of Health-System Pharmacists and the Pharmacy Society of Wisconsin. His areas of interest include medication safety, transitions of care, and advancement of hospital pharmacy practice through leadership, staff development, and pharmacy resident and student education. He currently serves as preceptor within the required financial rotation within the PGY2 Health System Pharmacy Administrative Residency.

    Justin is married to his wife (Katie) and has two sons, Cooper and Cole. The Guthman family enjoys spending time with outdoor activities including skiing, boating, biking, running and camping.

  • Elizabeth Papa, PharmD, CPPS
    Elizabeth Papa, PharmD, CPPS, earned her Doctor of Pharmacy degree from Thomas Jefferson University in 2014. She completed a PGY1 Pharmacy Practice Residency at Shore Medical Center in Somers Point, NJ and a PGY2 Medication Use Safety Residency at the Clement J. Zablocki VA Medical Center in Milwaukee.

    In July 2016, Elizabeth joined St. Joseph as the medication safety pharmacist and serves as preceptor for the medication safety rotation for PGY1 pharmacy residents and APPE students. She is a member of the American Society of Professionals in Patient Safety, American Society of Health-System Pharmacists, and Greater Milwaukee College of Clinical Pharmacy.

  • Kyle Piscitello, PharmD
    Kyle J. Piscitello, PharmD received his PharmD from the University of Wisconsin – Madison School of Pharmacy and completed a PGY1 pharmacy residency at Wheaton Franciscan - St. Joseph Campus in 2015 and a PGY2 infectious diseases pharmacy residency at UnityPoint Health Meriter in 2016.

    In 2016, Kyle returned to St. Joseph Campus as an antimicrobial stewardship pharmacist. In this role, he serves as a preceptor for the PGY-1 antimicrobial stewardship/infectious diseases required rotation in addition to a new role as the preceptor for the infectious diseases elective rotation for the Ascension Wisconsin Health-System Pharmacy Administration PGY-2 program.

    In addition to being a preceptor and his daily clinical practice, Kyle is also the co-chair for the North Market (Legacy) Antimicrobial Stewardship Committee and is a participating member of the North Market (Legacy) Infection Control + Clostridium difficle, Critical Care, and Residency Advisory committees.

    As of August 2017, Kyle was appointed as a pharmacy representative for the Ascension Wisconsin Infection Prevention Council. He is also a member of a number of professional organizations, including ASHP, ACCP, the Society of Infectious Disease Pharmacists, the Pharmacy Society of Wisconsin, and the Greater Milwaukee College of Clinical Pharmacy. Additionally, he will be sitting for the pharmacotherapy board certification exam in Fall 2017.
  • Andrew Ticcioni, PharmD, BCPS
    Andrew Ticcioni, PharmD, BCPS earned his Doctor of Pharmacy degree in 2011 from the University of Minnesota. He completed a PGY1 pharmacy residency at St. Joseph in 2012, and soon after achieved board certification in Pharmacotherapy. Following his residency, Andrew stayed at St. Joseph as an inpatient pharmacist. In 2013, he began serving as the primary preceptor for the Internal Medicine rotation and also became the Pharmacy Residency Program Coordinator. He teaches advanced cardiovascular life support for Wheaton and has interests in internal medicine, critical care, transitions of care and experiential pharmacy education. He primarily practices on the general medicine/surgical service and in the emergency department.

    Andrew is a member of the American Society of Health-System Pharmacists, Pharmacy Society of Wisconsin, American College of Clinical Pharmacy and is the Resident/Student/Fellow Committee Chair for the Greater Milwaukee College of Clinical Pharmacy.

    Andy enjoys learning new home improvement skills, spending time with his family, exploring the great city of Milwaukee and volunteering.
